In today’s fast-paced world, most people tend to pop a painkiller at the first sign of a headache, seeking quick relief without addressing the underlying cause. However, Ayurveda takes a holistic and root-cause approach by analyzing the imbalance of the three doshas—Vata, Pitta, and Kapha. According to Ayurveda, headaches are not all the same; they vary based on the aggravated dosha. Some may experience a hammer sensation pain due to Vata imbalance, others may feel a burning or throbbing pain  linked to aggravated Pitta, while a sense of heaviness and dull ache is often due to Kapha dominance. Instead of offering temporary relief, Ayurveda focuses on identifying the doshic imbalance and treats the headache accordingly with herbs, diet, lifestyle changes, and Panchakarma therapies. This individualized approach not only relieves pain but also prevents its recurrence, offering a long-lasting solution by healing from the root.

Shirahshool (Head Ache) Introduction

“शिरः शूलं कफ पित्त अनिलैर्वा सह संश्रितम् ॥”Charaka Samhita (Sutra Sthana 20/11)

Headache (Shirahshool) is caused due to the vitiation of Kapha, Pitta, and Vata, either individually or in combination.

Types of Shirhshool

1. Vataj Shirahshool 

Cause: Vata vitiation (due to fasting, excessive physical work, lack of sleep, cold exposure).
Symptoms: Pricking, splitting,pulsating pain, shifting in nature, aggravated in cold.

वातेन कारणेनैव शिरो वेदनमत्ययम्।

वेगेन कम्पते मुष्णन् भ्रमं जनयति क्वचित्॥माधव निदान – शिरोरोग निधानम् – श्लोक 7)

The headache caused by Vata dosha leads to intense pain in the head.The pain is sudden, severe, and pulsating, often accompanied by trembling (shaking).
It may cause dizziness or giddiness (bhrama).The nature of pain is variable, sometimes shifting in location, and worsens with cold or empty stomach

2. Pittaj Shirahshool 

Cause: Pitta imbalance (due to excessive heat, spicy foods, anger, sun exposure).
Symptoms: Burning pain, redness in eyes, thirst, sweating, light sensitivity.

पित्तेन कारणेनैव शिरो दहति सर्वदा।

लोचनयोश्च दाहः स्याल्लालास्रासो भविष्यति॥८॥माधव निदान – शिरोरोग निधानम् – श्लोक 8

When Pitta dosha is aggravated, it causes burning sensation in the head,throbbing pain.
There is burning in the eyes (लोचनयोः दाहः).Redness and watering of the eyes (लालास्रासः) are also common symptoms.The pain is typically intense, burning, and associated with heat, often aggravated by sun exposure, anger, or spicy food.

3. Kaphaj Shirahshool (कफज शिरःशूल)

Cause: Kapha vitiation (due to cold food, sedentary life, heavy meals).
Symptoms: Dull, heavy pain, feeling of heaviness, stiffness, nasal congestion.

शिरःस्तब्धं गुरु शीतं मन्ये दुःखं न वेगवान्।

प्रसक्तं निःस्वनं चैव कफजं शूलमुच्यते॥९॥माधव निदान – शिरोरोग निधानम् – श्लोक 9

In Kaphaja Shirahshool, the head feels heavy (guru), stiff (stabdham), and cold (śītam).
The pain is dull, not very intense, and non-throbbing (na vegavān).Stiffness in the neck (manya) and continuous noise/sound (niḥsvanam) in the head may also occur.
This type of headache is due to Kapha dosha, and is often aggravated by cold, damp weather, heavy food, or inactivity.

4. Sannipataj Shirahshool (संनिपातज शिरःशूल)

Cause: Tridosha involvement.
Symptoms: Mixed features of all doshas, chronic and severe pain.

सर्वलक्षणयुक्तं तु शोफहर्षविवर्जितम्।

दुर्विज्ञेयं च दुःसाध्यं त्रिदोषोत्थं शिरो रुगम्॥माधव निदान – शिरोरोग निधानम् – श्लोक 10

The Sannipataja Shirahshool is caused by vitiation of all three doshas — Vata, Pitta, and Kapha.It presents with mixed symptoms of all doshic headaches (sarva-lakṣaṇa-yuktam).There is no specific swelling (śopha) or tingling/numbness (harṣa), but general heaviness and severe discomfort.It is difficult to diagnose (durvijneyam) and hard to treat (duhsadhyam).

5. Krimija Shirahshool (कृमिज शिरःशूल)

Cause: Infestation of head region by microbes/worms.
Symptoms: Itching, crawling sensation, discharge, foul smell.

6. Suryavarta (सूर्यावर्त)

Cause: Predominantly Pitta; worsens with sun exposure.
Symptoms: Pain starts mild in the morning and increases with the sun, peaks at noon.

7. Ardhavabhedaka (अर्धावभेदक

Cause: Vata + Pitta (mostly)
Symptoms: Unilateral severe pain, pricking and splitting type, nausea.

अर्धावभेदकं तु स्यात्तीर्यग्दाहानिलात्कृतम्।
पार्श्वे शिरश्च बाधेत तीव्रं बाणैरिवार्दनम्॥१४॥

गात्रसादः स्वरभ्रंशो जिह्वासङ्गश्च जायते।
सप्ताहत्रयसंस्थं स्यान्मोहो वा मरणं ध्रुवम्॥१५॥माधव निदान – शिरोरोग निधानम् – श्लोक 14–15

Ardhavabhedaka is a condition caused by Vata and Pitta dosha.
It presents as a severe one-sided headache (ardha = half, bhedaka = splitting), like being pierced by arrows.The pain is intense and localized to one side of the head or temple.Associated symptoms:
Gātrasāda – Weakness of limbs
Svarabhraṁśa – Loss of voice or hoarseness
Jihvāsaṅga – Stiffness or difficulty in tongue movement
If untreated, it may persist up to three weeks, lead to delirium (moha), or even death (maraṇam dhruvam) in severe cases.

Treatment of all types of Shirahshool-

1. Treatment for Vataj Shirahshool

Shiroabhyang with Brahmi or Jatamansi and other vaat shamkak oil

Shirodhara

Basti Chikitsa – Matra  Basti with Dashmoola or Ashwagandha oil

Nasya – with Anu Tail or Shadbindu Taila

Formulations: vaat shamak aushad like –

Vatachintamani Ras

Brihat Vata Chintamani Ras

Shirahshooladi vajra

2. Treatment of Pittaj Shirahshool

Virechana – therapeutic purgation to expel Pitta
Nasya – cooling nasya with Ksheerbala Taila or Brahmi Taila or desi ghee
Shirodhara – with medicated milk or coconut oil
Herbs: Brahmi, Jatamansi, Guduchi, Amalaki

Formulations:Sutshekhar Ras

Godanti Bhasma

Avipattikar Churna

Pathyadi Kadha

3.Treatment for Kaphaj Shirahshool

Shodhana – Vamana (emesis) in indicated cases

Nasya – with Trikatu Taila or Anutaila

Dhoompaan

Herbs: Pippali, Shunthi, Haridra, Tulsi
Formulations:
Sitopaladi Churna
Trikatu Churna
Talisadi Churna
